We are pleased to hear about your interest in Calvary Child Care Center. We are required to have the following completed forms prior to your child's first day of attendance. The forms must be completed by the legal guardian(s).
- General Information
- Emergency Procedures and Permission
- Enrollment, Attendance, Field Trips and Information Release
- Health Care Summary (signed by health care provider)
- Immunization History
- Emergency Cards (two)
- $75.00 non-refundable registration fee
- First weeks tuition, which is also non-refundable
After we have received the required forms and payments, we will then schedule a pre-enrollment visit between the family and a program staff member.
You can print these forms, fill them out, then fax, mail, or hand-deliver to the Calvary Childcare office. Fax to: 763.545.6953.
