Registration

We are happy for your request to enroll your child in Calvary Hourly Child Care Center. Our admission policy requires us to have the following completed forms on the first day that your child visits the center. All forms must be completed by the legal guardian(s).

  1. Registration Form
  2. Emergency Card
  3. Emergency Procedures & Permission Form
  4. Immunization Record
  5. $25.00 non-refundable registration fee (one time fee)
  6. $30.00 non-refundable permanent reservation fee
    (for those with a permanent reservation)

The second time that your child visits, we must have:

  1. Health Care Summary - signed by the child's doctor.

We encourage you to inform us of any special concerns or individual habits that will help us know your child better, so that your child's stay is a happy one. Please call our office with questions or to arrange a tour, 763-545-9042.

Maximum stay of 5 hours in one day

Days:

Tuesdays, Wednesdays, Thursdays, Fridays

Ages accepted:

16 months through Kindergarten age

Rates:

2008-2009

Toddler (16 mos.-32 mos.)
PreSchool (33 mos. - Kind.)
2nd child (in the same family)

$7.65/hr
$6.95/hr
$5.90/hr

Reservations:

To accommodate families with different needs, we offer two types of reservations.

Permanent

You reserve for the same time/same day, every week for the school year.
(Reservations open in the spring for the next school year reservations)

On-Call

Reservations may be made up to two weeks in advance.
