Please review the following information carefully to ensure that your child is properly registered for Calvary Pre-Kindergarten.
Registration forms & fees
As a first step and to ensure that a space is reserved for your child, please submit the following to us as soon as possible:
* Registration form
* Permission authorization form
* Emergency information form
* $60 non-refundable processing fee
Your child is not registered nor is space reserved for him/her until we receive the above forms and fee. Our classes fill quickly, so register soon.
Medical & authorized pick-up forms
As a second step, please submit the following forms to us by your child's first day of class:
* Immunization form
* Health summary form
Your child will not be permitted to attend class without our having these forms on record.
The immunization and health summary forms require the signature of your child's doctor. Make an appointment now since the wait for doctor's visits can be long.
