The following forms and information are required to be fully registered:

• Parental Permission Release form signed with $200 deposit
• Covenant
• Medication Administration (Form H)
  
• Health History Form (Form J)
• Parental Travel Consent Form - notary required
• Copy of student's Health Insurance Card (front and back)
  
• Copy of current Passport
  

Space is limited to 55 students and 10 adult chaperones.  As of 11/30/11 @ 7:30 p.m. this trip is FULL and wait-listed.
